Lost "My Number Notice Card"?

WHEN YOU HAVE LOST "MY NUMBER NOTICE CARD"

Above is a My Number "Notice Card", precisely, the left image is the "Notice Card", or "tsuchi card (通知カード)", which was sent to each citizen of Japan last autumn by registered mail. If you cannot find your card or you are sure that you have lost it, you can request to reissue a new Notice Card.

HOW LONG DOES IT TAKE TO RECEIVE A NEW CARD?
A new "My Number Notice Card" will be sent out in three weeks after application by registered mail, addressed to the householder. Please note that the mail cannot be transferred to another address than the one you registered at City Hall.


WHERE AND WHEN CAN I APPLY FOR A NEW CARD?
Apply at Citizens Services Division at Tsukuba City Hall, Counter No. 13 or 14. "My Number Notice Card" reissue application is accepted every day from 8:30 am - 5:15 pm (up to 8:00 pm on Thursdays), except public holidays and the end and the beginning of the year.


WHAT DO I NEED TO PREPARE FOR APPLICATION?
1. If you come to City Hall to apply by yourself:
(1)Bring your Residence Card or passport.
(2) If you have lost your card outside home, you need to report to the police first. You will need the contact of the police station where you reported the loss, the lost property report number (ishitsu todoke juri bango遺失届受理番号) , and how you lost your card on the Lost Report of "My Number Notice Card" (available at City Hall). If you have lost it in a fire, you will need to submit the damage certificate (risai shomei-sho り災証明書) issued by Fire Station or City Hall.


2. If a representative comes to City Hall to apply:
The following items are required.
(1)Identification of the representative
(2)(Representative for a person age 15 and older): Letter of Attorney
(Representative for a person age under 15): A document which proves that the representative is a legal representative
(3)The original identification of the applicant who needs a new card.


3. Reissue Fee: 500 yen
*Those who apply for a new card because they have just moved in from abroad or have no more space to put additional required information on the card, reissue fee is not required.


WHEN THERE IS A RISK OF UNAUTHORIZED USE
The individual number (My Number) should not change in principle. However, when there is a risk of unauthorized use due to loss outside home, etc., you can request to change your individual number.

APPLICATION FOR MY NUMBER CARD
If you wish to apply for your "My Number Card", a plastic card with your photo, you can apply for one without requesting reissue of "My Number Notice Card", a green paper one. The first application fee for "My Number Card" is free.


PLEASE NOTE:
As of April 2016, it is taking months to issue My Number Card due to a great number of application, which is dealt with by Japan Agency for Local Authority Information System (J-LIS).
