Information on Year-End Tax Adjustments and Income Tax Return:
Issuing of Social Insurance Premium Deduction Certificate (Shakaihokenryo Koujyo Shoumeisho) (updated November 14, 2011)

To get deduction for social insurance premium when filing tax returns for prefectual and municipal tax, the following taxes paid between 2011 January 1 to December 31 will be calculated:

  • National Health Insurance
  • Health Insurance for the elderly
  • Nursing-care Insurance
  • National Pension


* The certificate will be issued under the name of the taxpayer (Head of the household for National Health Insurance). This certificate is the record of the amount paid and will not be issued if payment had not been made. The Social Insurance Premium Deduction Certificate (for National Health Insurance, Health Insurance for the elderly, and Nursing-care Insurance) will be sent out to the taxpayer in the form of a postcard at the end of January 2012.

* If you need to verify the amount sooner in time for the year-end tax adjustments, or if you have lost the certificate, it can be issued for free at the following locations:

  • Health Insurance and Pension Division
  • Senior Services Division
  • General Service Counters


Please bring a valid ID (driver's license or alien registration card) and personal seal. A proxy is necesary if you live in a separate household.
